On this page, you will find the resources and forms needed to complete the CHHS Summer Reading Program.  Simply click on each link to open the document*.  You may print off and complete the forms as needed.  If you do not have access to a printer, you may pick up copies of these forms in the CHHS Main Office during the summer.
